How To Apply

To become a new patient of Dr. Griner, you must complete an application process that typically takes 2-4 weeks.  You may apply by clicking on the link below.  This will take you to a page where you can print a paper application that you may drop off at any of our locations during normal business hours.

Stop! Don't complete the Initial Paperwork until after you receive a letter of acceptance from our office!

Once you have been accepted as a patient, you will receive an acceptance letter from our office.  This letter also contains a medical records release.  You must complete and return the Medical Records Release to our office before we can schedule your first visit.  You will also need to complete a new patient paperwork packet prior to your first visit.  These forms are available for download at the links below, or you may complete them when you arrive for your first visit.  You should also bring a photo ID and insurance card to this visit.

Please do not complete new patient paperwork prior to being accepted as a patient.  You must complete the approval process prior to submitting any further paperwork.
