Thank you for choosing Griner Medical Group for your medical care needs. Every staff member in our office is dedicated to excellence. We strive to provide efficient, comfortable care for our patients. Please review our operating policies to make the most of your appointment.

Prospective patients of Griner Medical Group must complete an application. We accept a limited number of patients so that we can maintain personalized care. Please complete the following application to gain full access to our main practice and two walk-in clinics. Applications are approved on an individual basis.

You will be notified if you are accepted into our practice. Once you have been accepted, please download and complete the Accepted Patient Initial Paperwork prior to your first visit. You must complete the approval process prior to submitting further paperwork. Do not complete this until you receive approval from our office.

General Operating Policies

Appointments

We do our best to accommodate your schedule; however, please understand that we may not be able to offer your desired appointment time when our schedule is full.

We utilize a call-reminder system to provide you with a courtesy call four days in advance of your appointment. Please ensure that the telephone number you leave with us is correct and working so that we may communicate with you regarding your appointment and other important medical information. If you are unable to keep your scheduled appointment, please let us know as soon as possible.

Missed Appointments

It is your responsibility to provide at least 24 hours notice if you need to reschedule or cancel a scheduled appointment. If you fail to provide this notice, you will be charged a $25 no-show fee that is not billable to your insurance company. Your chart will be flagged. Accounts that accumulate more than three no-shows may be removed as patients of the practice.

Contact Information

Please keep us up to date if your contact information changes so we have accurate information for your address, phone number, and emergency contact. Please notify us at your next visit or before if you experience a change to your contact information.

Medical Records

It is our policy not to release any medical records, including immunization records, without first having the patient complete an Authorization to Release Medical Records form. Any of our Check-In/Out staff members can assist you with this request. Please note it may take 24-48 hours (1 week for Leave/FMLA paperwork) to complete your request. In addition, it is our policy to charge a small administrative fee for such paperwork at the time of the request.

Patient Rights and Responsibilities

At your first visit, you will be provided with our practice’s HIPAA policy as it relates to the handling, distribution, and storage of your Protected Health Information, more commonly referred to as PHI. You will also be provided with a document to complete which tells our staff who may receive your PHI and how you would like our office to communicate important medical information and appointment reminders to you and your designated parties or Powers of Attorney. You have the ability to change/revoke your authorization at any time by completing another authorization form.

If you feel that your PHI has been inappropriately used, accessed, or released, you may file a complaint with the Privacy Officer, Brett Johnson. Please contact our office at 229 242-6061 and ask to speak with him. We take your concerns seriously, and our staff understands their obligation to protect the confidentiality of your PHI.
